Our in-home physician team can evaluate and detect subtle declines in patient condition before they become larger issues which could result in re-hospitalization. Our Physician Team will recommend treatments, prescribe certain medications and confer with the patient’s primary care physician and oncologist team to reduce the chances of a re-admission to the hospital.
